MVR Guidelines
Driver Motor Vehicle Report (MVR) Guidelines*
- Clear Driver
MVR shows no minor convictions and no at-fault accidents for the past three years and has no major convictions.
- Watch Driver
We define a Watch Driver as one who:
1) has a total of two (2) incidents (at-fault accidents or minor convictions) in the past
three (3) years; and
2) has no major convictions in the past three (3) years
A Watch Driver may be placed on quarterly or bi-annual reporting. This requires that a new MVR be ordered on a quarterly or semi-annual basis and reviewed for additional incidents.
- Additional incidents will be evaluated to determine if the driver is a 'Poor Driver'.
- Poor Driver
We define a Poor Driver as one who:
1) has one or more major convictions in the past three (3) years
2) Any combination of three or more incidents (at-fault accidents or minor convictions) in the past three (3) years.
A Poor Driver should be excluded from coverage where allowed by state law.
*Driver exclusion subject to state law.
Definitions:
Major Convictions including but not limited to:
Variations of state motor vehicle codes exist, but examples include the following;
- Driving while intoxicated or (DUI or DWI);
- Failure to stop and report an accident;
- Attempting to elude an officer of the law;
- Assault, manslaughter, or homicide
- Driving w/suspended or revoked license;
- Reckless driving;
- Speed contests, drag or highway racing;
- Major speeding conviction (20 mph or more over the posted speed limit where
specified on the MVR); and
- Driving while unlicensed.
Minor Convictions
Examples of minor convictions include the following:
- Minor speeding conviction (less than 20 mph over the posted speed limit where
specified on the MVR);
- Driving too fast for conditions;
- Careless or inattentive driving;
- Unsafe lane change;
- Failure to stop or yield the right of way;
- Following too closely (no more than 1 in a 3 year period); and
- Any standard moving violation that does not fall into the major conviction category
- At-Fault Accident
- Any motor vehicle accident arising out of negligent operation of the vehicle for which the driver was at fault. Also refers to any other vehicle accident where it cannot be determined with reasonable assurance that the driver was not at fault.

For all Commercial Auto accounts, Student Drivers and Non-Employee Passengers are not allowed.
- Prospects large fleets can be submitted to us.
- Fleets greater than 15 autos, up to 50 autos, can be submitted as well. The larger fleets
are subject to approval and review by underwriting management.
- Additional underwriting criteria will apply to the larger fleets including the need for safety controls,
loss prevention survey, description of large losses, additional lead time, coverage requests, competition, type of operation, vehicle schedule, and answers to any underwriting questions prior to quoting.
Types of Vehicles
- In addition to private passenger and light trucks, we are accepting prospects which
have medium trucks, heavy trucks, extra heavy trucks, tractors and trailers. We are
not a market for tractors hauling double or triple tandem trailers.
- The heavier vehicles are also subject to additional underwriting consideration as
outlined above for the larger fleets.

Auto Transporters
- This classification includes automobile haulers engaged primarily in the transportation of new and
used private passenger and light trucks.
- Radius up to 200 miles.
Ineligible Exposures:
o Transportation of salvage or dismantled vehicles
o Drive-away or vehicle delivery services requiring the use of CA2005 Drive-Away Contractors
o Rollback only operations
o Mobile Home/ Manufactured Home Toters
o Automobile brokerage
o New ventures - The account must have a minimum of 2 years verifiable loss history in their own
name. No exceptions will be made.
Cement Mixers, Ready Mix and Concrete Pumpers
- This classification includes cement mixers, ready mix and concrete pumper trucks and trailers, which
typically operate at or near construction sites.
- Radius up to 100 miles. If incidental exposure over 100 miles exists, submit to your underwriter.
Ineligible Exposures:
o New ventures - The account must have a minimum of 2 years verifiable loss history in their own
name. No exceptions will be made.
Contractors
- This classification includes general contractors such as building contractors, welders, vacuum trucks, septic tank services and street sweepers. Also included in this classification are artisan contractors such as plumbers, lawn service, painters, home remodeling, tree service, electrical contractors, cleaning services, exterminators, etc.
- Radius up to 200 miles. If incidental exposure over 300 miles exists, submit to your underwriter.
- If the account has vehicle(s) with booms or cherry pickers, these vehicles must be underwritten and
rated using the Cranes and Boom Trucks underwriting guideline.
- If the account has incidental dump vehicles, they must be underwritten and rated using the Dumping
Operations underwriting guideline.
Ineligible Exposures:
o Remediation/Spill Clean up Contractors and Asbestos Abatement Contractors
Cranes and Boom Trucks
- This classification includes vehicles with permanently attached cranes, booms or cherry pickers,
which typically operate at or near construction sites.
- The vehicles must be licensed for road use.
- The account must have general liability coverage in force with another carrier and we must be named as a certificate holder.
- Radius up to 100 miles. If incidental exposure over 100 miles exists, submit to your underwriter.
Ineligible Exposures:
o New ventures- The account must have a minimum of2 years verifiable loss history in their own
name. No exceptions will be made.
Dump Trucks
- This classification includes vehicles hauling coal, aggregates, dirt or other non-hazardous dump
commodities.
Ineligible Exposures:
o New ventures - The account must have a minimum of 2 years verifiable loss history in their own
name. No exceptions will be made.
o For Sand + Gravel, and Coal hauling risks, minimum is 3 years verifiable loss history in their own
name.
o Operations hauling hazardous materials or waste, or non-petroleum contaminated dirt.
Food Delivery
- This classification includes vehicles used by food manufacturers or wholesale distributors to transport their own raw or fmished product such as bakery, produce, beverage, water, ice and meat distributors. Radius up to 200 miles.
Ineligible Exposures:
o Ice cream vendor
o Pizza delivery
Funeral Operations
Ineligible Exposures:
o Funeral escort services (Non-owned vehicles)
Garbage Operations
- This classification includes contracted collection of garbage, trash, refuse or recyclables at residential, commercial or industrial sites for disposal or recycling.
- Roll Off Operations.
- Salvage operations including scrap dealers.
Ineligible Exposures:
o Vehicles operating as a trucker and not being used for residential, commercial or industrial pickup and disposal (e.g., transfer station to landfill or hauling bailed trash)
o For hire scrap metal haulers
o New ventures- The account must have a minimum of 3 years verifiable loss history in their own
name. No exceptions will be made.
o Accounts transporting Regulated Medical Waste or Biohazard Waste
o Any risk with active brokerage authority.
Household Goods Movers
- This classification includes vehicles owned by household goods movers.
Ineligible Exposures:
o Non-Owned Auto liability
o Risks operating over 200 miles
Mobile Business
- This classification includes businesses operating in or from their own vehicles, such as bookmobiles,
mobile medical units, mobile repair or service units, roadside auto service and mobile food vendor.
- The account must have general liability coverage in force with another carrier.
Ineligible Exposures:
o Ice cream vendor
o Pizza delivery
Specialized Delivery
- This classification includes vehicles owned by courier services, magazine and newspaper distributors
and armored cars.
Ineligible Exposures:
o Door to door newspaper delivery
o Non-Owned Auto liability
Trucking (0- 200 MILES)
- This classification includes for hire operations such as common, contract or exempt carriers.
Ineligible Exposures:
o Bulk (Tank) Hazardous Materials (greater than 119 gallons or 882 pounds). Incidental hazardous
materials such as cans of spray paint, household cleaners and batteries are acceptable.
o Double and Triple trailers
o Risks Transporting butane, propane, LPG or gasoline.
o Regulated Medical Waste or Biohazard Waste
o Risks hauling munitions or explosives, including liquid Hydrogen, Nitrogen, Chlorine, fireworks,
fuses, dynamite, Nitroglycerin, Ammonia Nitrate.
o Mobile Home or Manufactured Home Toters
o Risks transporting any amount of Class 1 Explosives, Class 2.3 Poisonous Gas, Class 6 Poisonous Gas
or Class 7 Radioactive
o Any risk hauling logs
o Any risk with active brokerage authority
o Drive-away contractors. (Any operation whose employee(s) drive non-owned vehicle(s) from one
destination to another while requiring the use ofCA2005 Drive-Away Contractors.)
o New ventures - The account must have a minimum of 3 years verifiable loss history in their own

Manufacturers, Wholesale and Retail Delivery (0- 200 MILES)
- This classification includes businesses that deliver their own commodities such as appliances, auto
parts, building supplies, furniture, office equipment and supplies.
- Department stores, catering services, dairies, private carriers, farmers and lumberyards are also
included in this classification.
Ineligible Exposures:
o Bulk (Tank) Hazardous Materials (greater than 119 gallons or 882 pounds). Incidental hazardous
materials such as cans of spray paint, household cleaners and batteries are acceptable.
o Double and Triple trailers
o Regulated Medical Waste or Biohazard Waste
